Mandy Kubis

Mandy serves as Nurse Practitioner for Western Illinois Home Provider Care. Her healthcare journey began in 2008 and has since accumulated a wealth of diverse medical experience including oncology, med-surg, and emergency room care. Mandy found her true calling for in-home care in 2015 when she became a Case Manager for Western Illinois Home Health Care. She became wound care certified (WCC) in 2019 and became a Family Nurse Practitioner (FNP) in 2023. 

In her spare time, she enjoys crafting, cheering on her children at their various sporting events, and traveling internationally with her husband.

I have Medicare, can I still utilize WIHPC?

Absolutely! In-home primary care visits are perfect for elderly patients who have difficulty with transportation, long waits in the waiting room or are too frail to leave their residence.

What is Direct Primary Care (DPC) used for, and why do you still need regular health insurance?

Your DPC Membership is ideal for managing and preventing a wide range of health conditions, including chronic diseases and acute illnesses by providing preventive care. While DPC covers a significant portion of your primary care needs, it is not a substitute for traditional health insurance. Traditional health insurance is crucial to cover unexpected or major medical expenses, hospitalizations, and specialized care.

What if I have a problem after regular office hours?

We have staff on call 24 hours a day / 7 days a week. If you need to speak to someone after normal business hours that cannot wait for the office to open, call our office phone number and you’ll be directed to our on-call staff member.

What if I need to cancel my appointment with my in-home provider?

We request that you give us 24-hour notice to cancel your appointment. Cancellations must be done by phone call or by leaving a voicemail during regular business hours. Please do not cancel via patient portal message.

How do I pay for home provider care services not covered by another payor source?

Invoices are sent and payment is due within 15 business days of the date of the invoice. Patients can pay via ACH, credit/debit card (at no charge), check or cash.

What can I expect as a patient of Western Illinois Home Provider Care?

You can expect personalized care to meet your unique needs, a care team that works with you to meet your goals, and to be listened to, respected, and treated like family.

What if I need a specialist or a hospital?

Our high quality, quick, convenient medical care will reduce or prevent the need for many hospitalizations and specialty referrals. If the need for a specialist or hospital does arise, we will closely coordinate with any outside specialists and hospitals to provide you with the best possible care. Since we are not affiliated with any large healthcare systems or medical corporations, we can provide a high-quality, free-market approach to health care decisions. You and your provider decide who will best serve your needs and seek out the proper care with specialists and providers we know and trust throughout West Central Illinois.

Can I get my prescriptions through Western Illinois Home Provider Care?

Absolutely—As a nurse practitioner, Mandy is capable of legally prescribing the medications needed for managing your care. Your prescriptions will be sent to the pharmacy of your choice, just as they are with any other provider. Many medications can be ordered at a discount and shipped directly to your house.
